2010 National Notable Achievement Awards Nomination Forms

Submission Guidelines
The document format instructions for submitting award nominations have changed to reflect the
agency standard. The requested document format for nominations is:
Margins: 1-inch margins
Default Font Type and Size: 12-point Times New Roman font
Length: no more than two (2) pages in length.
Please refer to the Nomination Instructions (20pp, 131KB) for full details on submitting a nomination.
Nomination packages should be submitted by April 23, 2010 (this has been extended from the original March 26 deadline) with a transmittal memo
signed by the Deputy Regional Administrator. A sample nomination transmittal memo (4pp, 54KB) is available for guidance. Signed PDF copies of award nominations may be submitted electronically to mendes.jeanette@epa.gov or by mail:
